Office Hours
Our office is open from 8:00 - 5:00 Monday through Thursday and 8:00 - 12:00 on Friday. We do not answer our phones from noon to 1:30 p.m. If you need to reach us after hours or on weekends, press option 6 and your call will be directed to our answering service. They will contact the doctor on call regarding your concerns. If you have a medical emergency, call 911 or go directly to the ER.

All appointments for both offices are made by calling 520-219-9125.

Appointments
Please arrive at least 15 minutes early. When visiting our office for an appointment you will need to bring the following:

  1. Referral from your Primary Care Doctor if needed. Have your doctor fax relevant information to 520-219-9130 ahead of time if possible.
  2. Driver's license
  3. Current insurance card
  4. Completed forms downloaded from this website
    • Patient registration
    • Patient history (If you are taking any medication, they must be listed with the strength and dosage
    • HIPPA privacy form

scheduling your surgery
Should you require surgery, one of our Surgery Schedulers will contact you with a surgery date and time. They will provide you with all the necessary information you need to get your surgery done. They will also set up your first post operative appointment at this time. We ask that you do not call them unless you have not heard from them within 5 working days.

Prescription refills
Prescriptions for medications prescribed by our physicians are to be obtained Monday through Thursday 8:30 a.m. to 4:30 p.m. This will give us time to locate your chart and contact the doctor regarding your refill. Please call us by noon on Friday if you are going to require a refill. Please have your pharmacy name and phone number available when you call. We do not fill your other physicians' prescriptions. Our answering service is not able to assist you with a prescription refill when we are not in the office.

Insurance copays
Your copay is due when you check in for your appointment. We take cash, check or debit/credit. If you are not insured your office visit is due on date of service. If you require surgery, our patient account manager will be happy to speak with you to set up a payment plan if required.

Ultimately, your insurance is your responsibility. We deal with hundreds of plans and do not know everything about your individual plan. It is your responsibility to make sure you use a laboratory, radiologist and any other testing facility that is covered by your plan. We will get an authorization for your surgery with Presidio Surgical Group. Other charges by other providers involved in your care will be billed to your insurance such as the facility, assistant surgeon and the anesthesiologist. We do not bill for these services.

Disability forms
There is a fee of $25.00 for us to fill out any form you provide regarding your work status.
